The Zhealus Patient Support module extends from the Recovery Intelligence structure using the same post-visit assessments.

Patients continue to provide brief recovery updates after discharge. Their responses provide both longitudinal recovery information and the patient-level signals to identify individual patients who may not be recovering as expected and may benefit from earlier follow-up.

We organize those responses using our Conservative Concern Criteria and produce a Daily Callback Report so the care team can prioritize attention where it may be most useful, rather than relying on broad, non-targeted follow-up. This creates a more selective and manageable approach to post-visit support.

The module is designed to fit into existing clinical operations with minimal disruption.

Zhealus Health provides the recovery information and prioritization; the organization maintains responsibility for clinical decisions, patient communication, and documentation.

The result is a more focused way to support patients after acute care while also building the recovery data needed for broader clinical learning over time.
